29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:    a) GSC Enterprises, Inc. located at 720 West Longview Avenue in Mansfield, Ohio; On or about December 18, 2017 the employer did not conduct an evaluation to determine the respiratory hazards associated with welding and machining steel and ductile iron. Employees were potentially exposed to metal fumes such as but not limited to, iron oxide, manganese, magnesium, and nickel.
