29 CFR 1926.1412(f)(1): 29 CFR 1926.1412(f)(1): The equipment was not inspected at least every 12 months by a qualified person in accordance with paragraph (d) of this section:    (a) Jobsite, Graham Road, Wexford, PA:  On or about December 20, 2017 the Altec Model  0945TR,  Serial #0395BA0496 digger/derrick had not been inspected since March, 2014.  The equipment was being used to lift a traffic signal pole into position when the winch rope broke and the load fell and struck and killed an employee.

29 CFR 1926.1425(e)(1):  No employee must be directly under the load:    (a) Jobsite, Graham Road, Wexford, PA:  On or about December 20, 2017 an employee was positioned under the load operating a guideline as the load was being hoisted into position.  The winch rope broke and the load fell striking the employee resulting in his death.
