29 CFR 1910.178(m)(5)(iii): When the operator of a powered industrial vehicle dismounted and was within 25 ft. of the truck that was still in view, the load engaging means were not fully lowered, controls were not neutralized, and/or the brakes were not set to prevent movement:       a)  Warehouse Dock- On or about December 21, 2017, the employer did not ensure that the forks on the Yale sit down LPG forklift, Serial #N446266, were fully lowered when the operator of the vehicle dismounted and was within 25 ft. of the truck that was still in view.
