29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition. Worksite at the Waggoner #3 well site, Noble County, OK. On or about November 02, 2017, the walking-working area around the well site was not kept in a clean and orderly condition. Metal fencing, sucker rods, hoses, oilfield pipe, and other oil/gas well tools and equipment were laying on the ground and exposing employees to a trip hazard and struck-by and caught-in hazards from the drive assembly and counter weights.

Worksite at the Waggoner #3 well site, Noble County, OK. On or about November 02, 2017 the employer failed to report an in-patient hospitalization of one employee within the 24-hour reporting requirement from the time of the hospitalization.
