29 CFR 1915.1000(a)(2): The employee's exposure exceeded the 8-hour Time-Weighted Average (TWA) exposure limit for that substance listed in Table Z in any 8-hour work shift of a 40-hour work week:  Establishment - Employee's exposure exceeded the 8-hour Time-Weighted Average (TWA) exposure limit for substances listed in Table Z with ineffective engineering controls and no administrative controls in place:  a) An employee conducting arc gouging, grinding and welding on the ship in the starboard aft fuel tank (a confined space) located in the shipyard was exposed to copper fumes at a level of 0.19 milligrams per cubic meter averaged over an eight hour time period; which is 1.9 times the permissible exposure limit (PEL).   b) An employee conducting arc gouging, grinding and welding on the ship n the starboard aft fuel tank (a confined space) located in the shipyard was exposed to iron oxide fumes at a level of 24.89 milligrams per cubic meter averaged over an eight hour time period; which is 2.5 times the permissible exposure limit (PEL).
