29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) On or about February 1, 2018: Three employees of Roofing Pioneers, LLC were observed operating Hitachi pneumatic nail guns when installing asphalt roof shingles and were not provided with eye protection. Employees were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about February 1, 2018: On a 7:12 pitch roof six employees of Roofing Pioneers, LLC were observed installing asphalt roof shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 11-feet fall hazard.
