29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)    On or about 2/7/2018, Press #11, Deck #2 did not have a guard over the ingoing nip created by the bull gear to the cylinder gear to the analox gear on the back side of the press while it was in operation.  b)    On or about 2/14/2018, Press #11, Deck #5 did not have a guard over the ingoing nip created by the print cylinder and analox roll from the aisle side of the press while it was in operation.  c)    On or about 2/14/2018, Press #11, Rewind station had pinch points created by chopper knife during turnovers along with pinch points created by turret and frame of machine as new rolls were started.
