29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Facility - On January 14, 2018 an employee fractured a hip falling from a ladder and had to be hospitalized.  The employer did not notify OSHA of the hospitalization until January 31, 2018.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  a) Maintenance Shed - On or about February 8, 2018 and at times prior, employees were exposed to electrical hazards in that an electrical outlet that was in a complete insulation did not have a cover plate.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) The Facility - On or about February 8 and at times prior, the employer exposed employees to chemical hazards from chemical such as but not limited to diesel fuel, gasoline, and various pesticides in that they failed to develop and implement a written hazard communication program.
