F/V Siberian Sea, Coastal Dock, case up area; on or about 07 February 2018 and times prior thereto, six separate guards measuring approximately six inches by eight inches were missing along the glazer roller assemble.  Guards were missing on the left and right hand side of the power transmission shafting. This condition exposed employees to a caught in hazard.

F/V Siberian Sea, Coastal Dock, Engine Room; on or about 07 February 2018 and times prior thereto, a Jet six-inch bench grinder, Model JBG-6A, Stock number 577101, had the tool rest adjusted to greater than 1/8 inch from the abrasive wheel. Distance was approximately 1/4 inch from the abrasive wheel. This condition exposed employees to a caught in hazard.
