29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  F/V Siberian Sea, Coastal Dock, case up area; on or about 07 February 2018 and times prior thereto, six separate guards measuring approximately six inches by eight inches were missing along the glazer roller assemble.  Guards were missing on the left and right hand side of the power transmission shafting. This condition exposed employees to a caught in hazard.

29 CFR 1915.134(b): Work rests were not kept a distance not to exceed 1/8 inch from the surface of the wheel.  F/V Siberian Sea, Coastal Dock, Engine Room; on or about 07 February 2018 and times prior thereto, a Jet six-inch bench grinder, Model JBG-6A, Stock number 577101, had the tool rest adjusted to greater than 1/8 inch from the abrasive wheel. Distance was approximately 1/4 inch from the abrasive wheel. This condition exposed employees to a caught in hazard.

29 CFR 1915.89(q)(1):  The employer did not conduct an audit of the lockout/tags-plus program and procedures currently in use at least annually to ensure that the procedures and the requirements of this section were being followed and to correct any deficiencies.    F/V Siberian Sea, Coastal Dock, engine room, on or about 07 February 2018 and time prior thereto; the employer did not ensure that the annual audit of the lockout/tags plus program and procedure was completed. This condition exposed employees to unsafe work practices.
