F/V Bering Prowler, OSI dock, upper aft weather deck, on or about 10 February 2018 and times prior thereto; the employer did not ensure that unguarded edges were effectively protected. An unguarded edge measuring approximately 9-feet above the lower surface, did not have guardrails. This condition exposed employees to a fall hazard.

F/V Bering Prowler, OSI dock, weather deck, on or about 10 February 2018 and time prior thereto; the employer did not ensure that rigging gear was inspected periodically. A four-bridle nylon messing lifting assembly; comprising of four nylon mesh strands with attached hooks; had one hook noticeably sprung. This condition exposed employees to a struck by hazard.
