29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Factory Deck:  On or about February 7, 2018 and at times prior thereto, facilities were not available for employees that mixed the corrosive all purpose concentrated cleaner "Red Hot."  This condition exposed employees to eye injuries.

29 CFR 1915.1026(d)(4)(i): Within 5 working days after making an exposure determination the employer did not individually notify the employee in writing of the results of the of that determination or post the results in an appropriate location accessible to all affected employees:  Factory Deck:  On or about February 7, 2018 and at times prior thereto, the employer had not notified employees required to perform hotwork on stainless steel processing equipment on the exposure determination.  This condition limited employee information on the their exposure to Chromium (VI) hazards.
