29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a. On or about February 13, 2018 at the above addressed jobsite, employees were exposed to falls while performing carpentry operations from a tubular welded frame scaffold, thirteen (13) feet above the ground, that did not have the working level fully planked.

29 CFR 1926.451(g)(4)(xv): Cross-bracing used in place of a mid-rail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a top-rail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a top-rail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The endpoints at each upright were more than 48 inches (1.3 m) apart. a. On or about February 13, 2018, at the above addressed jobsite, employees conducting carpentry operations from a tubular welded framed scaffold, approximately thirteen (13) feet above the ground, were not provided with a mid-rail, thereby exposing the employees to falls.
