(a) Machine Shop: On or about February 5, 2018 and times prior thereto, a ladder did not have a self closing gate or offset or some means of protecting employees from a fall. This exposed employees to 13 foot fall hazard. (b) Fishmeal bagging area: On or about February 5, 2018 and times prior thereto, a ladder did not have a self closing gate or offset or some means of protecting employees from a fall. This exposed employees to 10 foot fall hazard.

29 CFR 1910.29(b)(4): When the 200-pound (890-N) test load is applied in a downward direction, the top rail of the guardrail system deflected to a height of less than 39 inches (99 cm) above the walking-working surface; IQF area forward: On or about February 5, 2018 and times prior thereto, the wire rope rail around the hatch had over 12 inches of deflection when slight pressure was applied, This exposed employees to a 9 foot fall in to the hatch.
