29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Storage Area: On or about February 7, 2018 and at times prior thereto, a 3 X 2.5 inch opening existed in the main electrical panel for the facility.  This condition exposed employees to smoke-inhalation hazards from fires caused by arcing of the electrical equipment.

This conditions exposed employees to a fall hazard of 8 feet.

29 CFR 1918.65(h)(9): When lift trucks or other mechanically powered vehicles were being operated on open deck-type barges, the edges of the barges were not guarded by railings, sideboards, timbers or other means sufficient to prevent vehicles from rolling overboard:  Tongass Provider, Starboard Side:  On or about February 7, 2018 and at times prior thereto, during the operation of a Svetruck S1152, Number 631143 on the open deck of the barge, sufficient means were not provided to prevent the truck from rolling overboard.  This condition exposed employees to drowning hazards.
