29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    a) West side of production area; on February 21, 2018 and times prior the employer exposed the employees operating in the pallet building area to noise at levels of 71.8% (equivalent to an 8 hour time weighted average TWA of 88.0 dBA) which is 1.5 times the action level of 50% (equivalent to an 8 hour TWA of 85dBA).  Results were calculated for a 452 minutes sampling period.  The employer did not make audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels.    Gulley Pallet Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.95(c)(1), which was contained in OSHA inspection number 966650, citation number 1, item 1 and was affirmed as a final order on 06/02/2014, with respect to a workplace located at 2000 N Conception ST, Mobile, AL 36610.
