Main Street, Bowling Green, MO 63334, on or about 2/14/2018, workers were observed on a JLG Lift, Model: 800AJ, S/N: 0300060243, descending from heights of approximately 3 stories high and the operators were not tied onto the lift basket.  The workers were exposed to a fall hazard.

Main Street, Bowling Green, MO 63334, on or about 2/14/2018, workers were observed on top a commercial building, approximately 3-stories high, actively deconstructing the parapet wall, and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  The workers were exposed to a fall hazard.
