29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  On or about February 20, 2018 in the C-Fold Department at 2600 Northridge Drive in Kaukauna, the employer did not ensure three closed guards on the C-Fold PCMC Sawhouse machine were secure during tissue production; one sliding guard on the product outflow end and two guards on the product in-feed end.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    On or about February 20, 2018 at 2600 Northridge Drive in Kaukauna, the control of hazardous energy procedure (lockout / tag out) for Paper converting machines IF1, IF2, IF3, and C-Fold did not include the following elements:       1.  Specific statement of the intended use of the procedure.  IF3 machine         2.  Specific procedural steps for the placement, removal, and transfer of lockout devices or tag out devices on energy isolating devices and the responsibility for them.  C-Fold machine

29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment.  On or about February 20, 2018 in the C-Fold Department at 2600 Northridge Drive in Kaukauna, employees were opening a sliding guard on the C-Fold Sawhouse machine and reaching into the machine to remove jams without using any lockout procedure; no alternative measures to provide effective protection were provided for the cleaning task.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    On or about February 20, 2018 at 2600 Northridge Drive in Kaukauna, the employer failed to conduct periodic inspections to ensure the procedures were accurate and employees understood their responsibilities under the energy control procedure being inspected.
