29 CFR 1910.176(b): Storage of materials stored in tiers were not stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. This violation occurred on or about February 23, 2018, in the yard, concrete skids were stored at an excessive height and on cribbing that could not support the weight of the skids exposing employees to struck-by hazards.

29 CFR 1910.178(l)(2)(ii): Powered Industrial Truck training did not consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. This violation occurred on or about February 23, 2018, in the yard, employees who had not been adequately trained in the safe operation of powered industrial trucks, were carrying large unsecured loads with a Komatsu forklift exposing employees to struck-by hazards.
