29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  (a) Quencher & Draw Loader Conveyer (Hot Coil Quench) - On or about February 10, 2018 an employee  worked on the conveyer while pneumatic power was still applied. The lockout device was not placed on all air valves.
