29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. a) At 6232 Fly Road, East Syracuse, NY., on or about 2/27/2018: The employer permitted ladder use without ensuring safe access onto the roof, as the side rails of the ladder were positioned approximately 5 inches below the upper landing surface, exposing workers to fall hazards of 12 feet to the ground.
