29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a- 66 & 68 Havens Avenue, Lakewood NJ: Employees, not using head protection, were exposed to struck by injuries when working beneath workers using unsecured materials and equipment.  Condition observed on or about 02/27/18.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 66 & 68 Havens Avenue, Lakewood NJ: Company workers, not using fall protection, were exposed to fall hazards in excess of 14 feet to the ground surface below when walking/working at unprotected edges of a residential structure's first floor.  Condition observed on or about 02/27/18.   b- 62 Havens Avenue, Lakewood NJ: Company worksite employees, not using fall protection, were exposed to fall hazards in excess of 15 feet to the ground surface below when walking/working next to unprotected edges of a residential structure's first floor.  Condition observed on or about 02/27/18.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
