29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    a) 259 W. 10 St. apartments on 2nd floor:  Employee was exposed to electric shock while using an electric grinder that was not protected by a GFCI or an assured equipment grounding conductor program, on or about 3/6/18.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladders wern't secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) Exterior of Building - Roof Right Side: Employees engaged in siding activities were exposed to fall hazards of approximately 15 feet while utilizing a ladder that was not secured at the top, on or about 3/6/18.  a) Exterior of Building - Roof Back: Employees engaged in siding activities were exposed to fall hazards of approximately 12 feet while utilizing a ladder that was not secured at the top and did not extend at least 3 feet over the landing surface, on or about 3/6/18.
