On or about March 16, 2018 at 4889 S. Royal Atlanta Road, Tucker, Georgia, an employee operating a cherry lift was exposed to fall hazard, when the employee was not trained to recognize hazardous conditions including, but not limited to the following:  a.  when wearing the full body harness, the employee did not position the leg straps of the lanyard around his legs.      b.  the 8 foot lanyard used as part of the fall protection was wrapped around the bars that made up the ROPS to make a shorter lanyard   c.  the end of the lanyard that should have been attached to the anchorage point provided on the cherry picker was attached to one of the bars on the ROPS in such a way that the hook on the lanyard would not close and lock in the way it was designed.
