(a) Terminal 6 Berth 601 - M/V Silver Ray - On or about March 13th, 2018 and at times prior thereto, the employer failed to ensure all employees operating motor vehicles, during longshoring operations, were wearing restraint devices.

29 CFR 1917.26(f): A U.S. Coast Guard approved 30-inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees' work exposes them to the hazard of drowning.  (a) Terminal 6 Berth 601 - M/V Silver Ray - On or about March 13th, 2018 and at times prior thereto, the employer did not provide a life ring with at least 90 feet of line.  There was approxiamtely 75 feet of line on the life ring located on the eastern side of the pier where M/V Silver Ray was located.
