29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity:  a) On or about March 15, 2018, in the fabrication yard area, the employer failed to ensure that the conductors were not attached to terminals or leads so as to reverse designated electrical polarity. An extension cord with a shop-made 4x4 box with multiple adaptor had a receptacles wired with the designated polarity reversed, as the neutral connector was hot.

A flexible electrical extension cord with a 2x4 box had an open ground connection.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) On or about May 30, 2018, in the fabrication yard area, the employer failed to ensure that the pedal controls of the Scotchman DO-70/110-24M shear/forging machine were connected to the strain release devices. The pedal control cable's protective sheet were not connected to the strain release.   b) On or about March 15, 2018, in the fabrication yard area, the employer failed to ensure that a flexible electrical extension cord with a 2x4 utility box used as multiple connector was connected to the strain release devices. The utility electrical box cable's protective sheet was not connected to the strain release.
