29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  On or about March 13, 2018, at the front of the newly constructed residential home located at 14 Waterside Drive in Little Rock, Arkansas,     (a)  An excessive accumulation of debris such as but not limited to scrap sheathing, wooden boards and roof shingles were scattered on the ground.  This condition exposed employees to slips, trips and falls from the same elevation.  (b)  Wooden boards with protruding nails were scattered on the ground.  This condition exposed employees to punctures.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about March 13, 2018, at the worksite located at 14 Waterside Drive in Little Rock, Arkansas, employees did not don eye protection while using pneumatic nail guns to fasten shingles to the roof.  This condition exposed employees to the hazard of eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 13, 2018, at the worksite located at 14 Waterside Drive in Little Rock, Arkansas, employees on a steep pitched roof engaged in roof shingling installation tasks were exposed to unprotected falls of greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 13, 2018, at the worksite located at 14 Waterside Drive in Little Rock, Arkansas, the portable ladder used to access and descend from the roof did not extend 3 feet above the landing surface.  This condition exposed employees to falls from elevations.
