OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to chemical burns and inhalation hazards from an uncontrolled release of anhydrous ammonia:  a. South Engine Room - On or about March 15, 2018, the ammonia piping mains, headers and branches were not marked ammonia and identified as to the physical state of the refrigerant, the relative pressure level of the refrigerant, and the direction of flow.  Reference:  IIAR 2-2014, Section 5.14.5.    b. North Engine Room - On or about March 15, 2018, the ammonia system king valve (NV1 008) was not clearly and uniquely identified as an emergency shutdown valve.  Reference:  IIAR 2-2014, Section 5.14.3.    c. South Engine Room - On or about March 15, 2018, the +15 degree F Intercooler oil pot (S-OP-1) nameplate did not provide the manufacturer's information and design limits and purpose.  Reference:  IIAR 2-2014, Section 5.14.4.1.    d. South Engine Room - On or about March 15, 2018, the ammonia system king valve (S3V 007) was not directly operable or chain-operated from the work surface.  The king valve was located approximately 82 inches (6 feet, 10 inches) above the floor.  Reference:  IIAR 2-2014, Section 6.3.3.2.    e. North Engine Room - On or about March 15, 2018, the ammonia system king valve (NV1 008) was not directly operable or chain-operated from the work surface.  The king valve was located approximately 96 inches (8 feet) above the floor.  Reference:  IIAR 2-2014, Section 6.3.3.2.    f. South Rooftop Evaporative Condenser System - On or about March 15, 2018, the ammonia relief vent discharge was 68 inches (5 feet, 8 inches) above the highest platform accessed by employees during service and inspection.  Reference:  IIAR 2-2014, Section 15.5.1.4.    g. North Rooftop Evaporative Condenser System - On or about March 15, 2018, the ammonia relief vent discharge was located below the highest platform accessed by employees during service and inspection.  Reference:  IIAR 2-2014, Section 15.5.1.4.    h. South Rooftop Evaporative Condenser System - On or about March 15, 2018, the termination of the ammonia relief vent discharge was directed downward and not arranged to avoid spraying ammonia on to employees in the vicinity during service and inspection. North Rooftop Evaporative Condenser System - On or about March 15, 2018, the termination of the ammonia relief vent discharge was directed downward and not arranged to avoid spraying ammonia on to employees in the vicinity during service and inspection.
