29 CFR 1910.38(c)(2): Procedures for emergency evacuation, including type of evacuation and exit route assignments, were not included in an Emergency Action Plan.   On or about March 29, 2018: Employees working in the warehouse area of the facility are exposed to struck-by hazards where the procedures to safely utilize the fire lane was not included in the emergency action plan.  On March 12, 2018 an employee was hospitalized with a fractured left foot when she was struck by a turret truck moving down aisle 5 when she was walking through the fire lane.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.    On or about March 20, 2018: Employees working in the facility are exposed to trip hazards and electrically related hazards where the electrical panel boxes are blocked by debris to include old lockers and boxes.
