In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy: Dayhab Room: On or about March 23, 2018, and at times prior thereto, employees were exposed to electrical hazards while plugging equipment to outlets that had a missing cover plate exposing electrical conductors.
