Materials and equipment were placed, either permanently or temporarily, within the exit route:    a) In the back of the facility near the Men's Bathroom: On or about March 23, 2018, at times prior thereto, the fire exit door was locked, creating an egress hazard for employees in the event of an emergency.     b) In the back of the facility near the Women's Bathroom: On or about March 23, 2018, at times prior thereto, the fire exit door was locked, creating an egress hazard for employees in the event of an emergency.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose.  In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy:    Dayhab Room: On or about March 23, 2018, and at times prior thereto, employees were exposed to electrical hazards while plugging equipment to outlets that had a missing cover plate exposing electrical conductors.
