29 CFR 1910.67(c)(2)(v): The employer did not ensure a personal fall arrest or travel restraint system that meets the requirements in subpart I was worn and attached to the boom or basket when working from an aerial lift:      On or about April 3, 2018, an employee was exposed to a fall hazard of approximately 15 feet while in the basket of a vehicle mounted aerial lift  and the employees personal fall arrest or travel restraint system was not attached to anchorage point on the basket.
