29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. a) Jobsite: Employees erecting a temporary structure were working from temporary elevated platforms supported by a Moffett Truck-Mounted Forklift specifically not designed by the manufacturer for such use. The employees were installing a vinyl cover on the roof of the temporary structure, approximately 8 feet above ground level. Observed on or about 4/6/18.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a) Jobsite - The employer did not ensure that each employee was competent to operate a powered industrial truck safely through the successful completion of a powered industrial truck operator training program and as referenced by the note contained in 1926.602(d). The operators of the Moffett Truck-Mounted Forklift were not familiar with the truck's load stability data. b) Jobsite - The employer did not ensure that each employee was competent to operate a powered industrial truck safely through the successful completion of a powered industrial truck operator training program and as referenced by the note contained in 1926.602(d). An operator dismounted the Moffett Truck-Mounted Forklift without fully lowering the load engaging means, neutralizing the controls, and setting the brake. The operator elevated a platform approximately 8 feet above ground level, dismounted the truck, and accessed the elevated platform to assist a co-worker install a vinyl roof cover on the roof of a temporary structure. Observed on or about 4/06/8.
