29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as minor tool changes and adjustments, and other minor servicing activities. Employees were exposed to amputation, crushed-by, and struck-by hazards. The employer had not developed energy isolation alternative measures procedures to prevent unexpected energizing or release of stored energy. Press operators were required to adjust and change dies, without using lockout tagout procedures on equipment such as but not limited to the Minster Press serial number 70-5-7097.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Employees were exposed to amputation, crushing and struck-by hazards. The employer had not developed machine specific lock out procedures for isolating potential energy of equipment and provided no training to, or periodic reviews of authorized employees. Equipment included but was not limited to the press, known to the company as Minster 70-5-7097.

29 CFR 1910.217(e)(1)(i): Periodic and regular inspections of each power press were not conducted to ensure that all of its parts, auxillary equipment, and safeguards, including clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment. The employer did not maintain a certification record of each inspection, and each maintenance and repair task performed, under the general component of the inspection program that includes the date of the inspection, maintenance, or repair work, the signature of the person who performed the inspection, maintenance, or repair work, and the serial number, or other identifier, of the power press inspected, maintained, and repaired. Press operators are exposed to caught-in and struck -by injuries, when operating mechanical power presses without periodic or regular inspections, to ensure presses are in a safe operating condition and adjustment. Including but not limited to the Minster press known to the company as machine 70-5-6912.
