Site: 150 5th Avenue New York, NY  On or about 4/10/18  a) Employer did not ensure a competent person provided frequent and regular inspections of the jobsite, materials and equipment.  An employee working from an 8 ft. A-Frame ladder was injured when a section of HVAC duct fell striking the employee and the ladder.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  Site: 150 5th Avenue New York, NY.  On or about 4/10/18  a) The employer did not report within 24-hours a work related incident when an employee fell from a ladder and was hospitalized for injuries.  The incident occurred on 08/10/2018 and OSHA was notified on 08/12/2018. Repeat 1  Force Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(b)(2) which was contained in OSHA inspection number 1147045, citation number 2, item number1 and was affirmed as a final order on 1/3/18 with respect to a workplace located at 551 Waverly Avenue Brooklyn, NY.
