29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)  On April 11, 2018, and at times prior thereto, employees were not provided protective helmets while working below employees on the roof utilizing small hand tools exposing them to overhead hazards.  (b)  Garage: On April 11, 2018, and at times prior thereto, an employee was standing at the base of a ladder while another was working elevated utilizing small hand tools and exposed to an overhead hazard.  Note: Abatement certification IS required this item.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  On or about April 11, 2018, employees installing solar panels on a residential structure had donned full body fall protection harnesses but anchored to the side/hip D-ring (fall restraint) instead of connecting to the dorsal D-ring (Fall Arrest) exposing them to an approximate fall hazard at varied heights between 10-18 foot fall hazards.  b)  On or about April 11, 2018, an employee observed on the roof was not anchored while involved in solar panel installation.  Note: Abatement certification IS required for this item.
