29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a)     On or about April 18, 2018 and at times prior, employees were not provided with effective hazard communication training when required to use hazardous chemicals including, but not limited to, Gunk Swab Concrete Degreaser, naptha, heptane and methanol.     Abatement Note:  Abatement certification is required for this item (see enclosed Sample Abatement-Certification Letter).

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a)  On or about April 18, 2018, and at times prior, employees were exposed to eye injuries while they used corrosive chemicals such as Gunk Swab Concrete Floor Degreaser without access to a suitable eyewash facility in the immediate area.      Lone Star Investments, LLC dba Big O Tires was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 887823 citation 1 item 6, and became a final order January 13, 2014, with respect to a workplace located at 650 Highway 105, Monument, CO  80132.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed Sample Abatement-Certification Letter).
