29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced at the lifting device before it is lifted more than a few inches as required.    (a) On or about April 17, 2018, an employee moving an empty furnace basket with a jib crane and hoist did not insure the load was well secured and properly balanced. This exposed the employee to caught in, caught between and crushing hazards.

29 CFR 1910.179(n)(3)(v): Employee(s) were on the load or hook while hoisting, lowering or traveling was performed:    (a) On or about April 17, 2018, an employee(s) moved an empty furnace basket using the Gorbel 1,000 pound capacity Jib Crane and CM Value Star 1/2 ton capacity hoist.  The employee used hands to guide the hoisted basket while traveling and lowering to its resting place on the roller conveyor. This exposed the employee(s) to caught in, caught between and crushing hazards, and amputation injury.
