29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a)Limoges Construction, Inc. at 1121 North Covell Avenue, Sioux Falls, SD 57104: On or about and at times prior to April 24, 2018, the exposing employer did not ensure employees were protected from hazards associated with the operation of the Stihl TS 700 abrasive cut-off machine in that, the employer did not instruct employees in the recognition and avoidance of unsafe operating conditions. On April 24, 2018, an employee utilizing an abrasive cut-off machine was attempting to cut through rebar horizontally and was burned severely as a result. The sparks generated from the cut-off machine contacted the employees clothing causing them to ignite. The employer did not provide training to an employee involving proper body placement during use, correctly fastening the fuel cap on the machine to prevent fires, and not utilizing the machine to make horizontal cuts which increases the lateral pressure that the manufacturer identified must be avoided. This condition exposed employees to fire hazards and struck-by hazards associated with operation of the Stihl TS 700 abrasive cut-off saw.
