Guardrail systems;  B.  Safety net systems or  C.  Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:    Epsom Building, Second Floor, Crystalizer Area.  On or about May 2, 2018 employees were exposed to falls greater than 4 feet to the first floor below.  The guardrails surrounding the floor holes around the crystalizer machine were not continuous.  Employees used the lower rungs of a step ladder near the opening in the guardrails to reach the top of a filter for filter changes.      There is no abatement documentation or certification required for this item.
