29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.     On or about May 16, 2018, employees working from the interior of the 4th floor of an apartment complex were exposed to a fall from a window opening approximately 34 feet above the ground level below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about May 16, 2018, the employer did not ensure that each employee exposed to fall hazards on the worksite was trained in procedures to be followed to minimize fall hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about May 16, 2018, the employer did not maintain certification of training for each employee exposed to fall hazards on the worksite.
