29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about May 22, 2018, at a building renovation site located on corner of NW 63rd and May in Oklahoma City, OK, an employee was allowed to work in the basket of a aerial lift without fall protection, exposing the employee to the hazard of falls.
