29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 05/24/18, on the north side of a house at 231 North Park Ave., Buffalo, NY: Employees were standing, walking, and working from the aluminum "pic" of a ladder jack scaffold. The employees were exposed to a fall of approx. 18 ft. to the ground. No fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED

NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 05/24/18, at a roofing  jobsite, on the north side of a 2-story house at 231 North Park Ave., Buffalo, NY: An employee engaged in roofing work on a steep roof was exposed to a fall of approx. 20 ft. No fall protection system was utilized. Pitch of the roof was approx. 6 in 12.  NO ABATEMENT CERTIFICATION REQUIRED
