29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    At 858 Acid Mine Road, Sullivan, MO, specifically inside of the Machine Shop, a blue boom was attached to the Toyota Forklift, m/n 7FGU35, s/n 61859, for the purpose of transporting material.  The manufacturer's written permission to attach the boom was not obtained in advance.  This violation was noted on 06/05/18.
