29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: On or about May 31, 2018, employees were exposed to a fall hazard of approximately 11 feet while unloading insulation equipment and materials near a wall opening and suitable fall protection was not used.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about, May 31, 2018, employees were exposed to a fall hazard up to 11 feet while unloading insulation equipment and materials near a wall opening and the employees were not provided training which would have enabled them to recognize the hazards of falling and provided them with the procedures to follow to minimize the hazards.
