29 CFR 1910.1048(c)(1): Employees were exposed to an airborne concentration of formaldehyde which exceeded 0.75 parts per million, as an 8-hour time weighted average:  For the employees spinning molten glass into a fiber onto the spinning drums using chemicals such as, but not limited to, Casco-Resin (TM) 9-4090 containing 0.1-0.2% Formaldehyde to bind the glass fiber on or about June 6, 2018 and times prior there to, were exposed to 1.63 parts per million of formaldehyde calculated as an 8 hour time weighted average.

29 CFR 1910.1048(m)(1)(iii): The employers did not include formaldehyde in the hazard communication program established to comply with the HCS (� 1910.1200). Employers did not ensure that each employee had access to labels on containers of formaldehyde and to safety data sheets, and was trained in accordance with the requirements of HCS and paragraph (n) of this section.    For the employees handling the resin containing 0.1-0.2% Formaldehyde on or about June 5, 2018 and times prior there to, the employer did not ensure all employees working with or near the resin received and comprehended the training on the hazards of the resin and how to protect themselves and how to identify the signs and symptoms of exposure to formaldehyde.
