29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    (a) American Dream Construction at 41543 HWY 133 Paonia, CO 81425: On or before 6/5/18 the employer did not protect employees from injury in that employees rode a home made man basket mounted on a rough terrain forklift that was not designed by a manufacturer to serve specifically as a man basket. This condition exposed employees to a fall hazard of 17 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) American Dream Construction at 41543 HWY 133 Paonia, CO 81425: On or before 6/5/18 the employer did not protect employees from injury in that employees installing ridge cap on a new single family home were not wearing fall protection. This condition exposed employees to a fall hazard of 17 feet.    (b) American Dream Construction at 41543 HWY 133 Paonia, CO 81425: On or before 6/5/18 the employer did not protect employees from injury in that employees riding a man basket mounted on a rough terrain forklift were not wearing fall protection and not using guard rails. These conditions exposed employees to a fall hazard of 17 feet.
