29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were independent of any anchorage being used to support or suspend platforms but were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached:  a. On May 31, 2018, on the 6:12 pitch roof, the fall protection anchor was installed in weakened plywood, causing it to be ripped loose from the plywood when the employee fell, exposing the employee to a 19 feet fall hazard.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  a. On June 4, 2018, the employer did not timely report that an employee was hospitalized from a workplace accident that occurred on May 31, 2018.
