29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by a pinch point:     At 1000 Integram Drive, Pacific, MO, specifically inside of the Bundler Line, the support bar at the base of the Ground Lift Scissors Table, m/n EHLTG-4450-2-36, was unguarded.  One employee received an amputation of the big toe and part of the middle toe on or about 05/31/18.       PLAZE INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 937827, citation number 2, item number 1, and was affirmed as a final order on 10/30/13, with respect to a workplace located at 105 Bolte Lane, St. Clair, Missouri 63077.
