On or about June 19, 2018, at the service area building, at Yacht Haven Grande. Employee installing metal roofing 22 feet above ground was exposed to fall hazards while working on the roof; and also when gaining access to and from the roof by climbing the frames of the tubular welded frame scaffold. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a. On or about June 19, 2018, at the service area building, at Yacht Haven Grande. Employee was installing metal roofing when he fell 22 feet to the ground below. The lanyard (rope) he was attached to broke 10 feet 5 inches from the anchorage point, causing employee to fall onto the ground below. The rope had multiple kinks throughout, showed an appearance of being worn and was not removed from service. Employees were not adequately trained. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
