29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative: a) South side of the shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to struck-by hazards while servicing and maintaining the Fadal hydraulic CNC vertical machining mill, without a lockout-tagout program in place. b) Rear of the shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to struck-by hazards while servicing and maintaining the Hercules shear, without a lockout-tagout program in place. c) Center of the shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to struck-by hazards while servicing and maintaining the Accurpress Press Brake , without a lockout-tagout program in place.

On or about June 13, 2018, and at times prior thereto, employees were exposed to caught in- between hazards while operating a Japan Machinery and Metal (JMI) lathe, Model 14524, without proper guarding measures. b) South side of shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to caught in- between hazards while operating a Fadal CNC vertical machining mill, Model No. 906-1, Serial No. 9609407, without proper guarding measures. c) North side of shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to caught in- between hazards while operating a Katukura Turret Lathe, Model 7L-T without proper guarding measures. d) Rear of shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to caught in- between hazards while operating a Hercules shear Model S500-120, A4595, Serial No. 3751206574117, without proper guarding measures. e) Center of shop. On or about June 13, 2018, and at times prior thereto, employees were exposed to caught in- between hazards while operating a Accurpress Press Brake, Model 717512, without proper guarding measures. f) Front of shop. The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch: Front of shop. On or about June 13, 2018, employees were exposed to a struck-by hazard while deburring metal parts, as the distance from the wheel to the tongue of the bench grinder exceeded one-fourth inch.
