29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: (a)Siteworks, LLC at the Intersection of Arrowhead Parkway and South Foss Avenue, Sioux Falls, SD 57110: On or about and at times prior to June 14, 2018, the exposing employer did not ensure employees were protected from cave-in and struck-by hazards in that, the ladder used to access the excavation was located between the exterior wall of the trench shield and the excavation wall during use. This condition exposed employees to cave-in and struck-by hazards while working in a 12-foot deep excavation.

29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: (a)Siteworks, LLC at the at Intersection of Arrowhead Parkway and South Foss Avenue, Sioux Falls, SD 57110: On or about and at times prior to June 14, 2018, the exposing employer did not ensure employees were protected from cave-in and struck-by hazards in that, an employee performed work within the Efficiency Production Hydraulic Aluminum Shield Model #HS810 without all the spreader bars being installed. This condition exposed an employee to cave-in and struck-by hazards while working in an trench excavation 12 feet in depth.
